What is an affidavit of service — or a due diligence affidavit? An affidavit of service is a sworn, notarized statement confirming when, where, how, and to whom a document was delivered. If good-faith attempts don't result in service, a due diligence affidavit instead documents every attempt made, which counsel can use to request alternate service from the court. Every job through On Notice includes one or the other.
